There is no shinkansen direct to Tokyo . You will have to take the Shinkansen from Tokyo to Hachinohe and change to a rapid express train to Hakodate and then transfer up to Sapporo. The cost of all these trains are covered by your rail pass. It will take you about 10 hours to do this, with short transfer times between the trains. From December this year the shinkansen will run all the way to Aomori which should cut the time a little.
The cost of the fare should be quite similar to a plane ticket; around ¥24,000 JPY; around $240 USD or € 161 EUR.
For that you can get a personal cabin where you can sleep. They serve dinner, have bathrooms etc. And you can see the view as you travel around.
