They first contact the embassy, then they ask Sushma. Embassy procedures take time, which the patients don't have. They want preferential treatment depending upon their condition. Indian embassy cannot put a doctor too see who needs immediate medical treatment and who doesn't. Then there are people who takes everything to twitter, that makes her impossible to see who needs more attention and who doesn't.

Hence, people with serious illness ask FM to endorse their case (That's what I call helping). Hence make the paper works go faster and get preferential treatment from others who are waiting visa apporval (Also since visa granting is a long procedure for Pakistani and vise versa). I do understand this is not charity.
