Being major of a city even if the most important city of the country, is not the same as being the Prime Minister of the country. The worries of farmers in Hokkaido or fishermen in Okinawa are not the same as the worries of office workers in Tokyo. If he was popular a national level he well be Prime Minister but he is not.
