What about the ship's radar? Even civilian radars designed for recreational boats can display the speed and course vectors of any approaching vessel automatically. For example, Furuno’s Doppler system automatically changes a target’s color depending on the threat level. Anything stationary or moving away from the boat is green. Targets moving toward your boat at 3 knots or more become red. The red targets also get an echo trail, and an audible alarm is sounded if the threat of collision is high.
Navy experts are equally stumped as to how this can happen.
"When you are going through the Strait of Malacca, you can't tell me that a Navy destroyer doesn't have a full navigation team going with full lookouts on every wing and extra people on radar," Jeff Stutzman, an ex-information warfare specialist in the Navy who works at Wapack Labs, told McClatchy.

“Collisions like these are extremely rare and two in one summer, both from 7th Fleet based in Japan, is stunning,” David Larter, a US navy veteran told The Guardian.
“The number of breakdowns that have to occur for something like this to happen make them a rare occurrence. Sailors monitor radars round the clock, they have multiple sailors standing watch on the bridge which also has a radar, and they have at least one lookout posted at the back end of the ship to watch for exactly these kinds of situations,” Larter continued.

The United States Navy will remove from duty the commander of the 7th Fleet amid the latest incidents involving four collisions in Asia, CBS News has learned.
Vice Adm. Joseph Aucoin -- a three-star commander of the U.S. 7th Fleet based in Yokosuka, Japan -- will be relieved of his duty Wednesday for loss of confidence in the wake of four accidents.
An announcement will be made by the top U.S. Navy commander Adm. Scott Swift early Wednesday, CBS News' national security correspondent David Martin reports.
The Wall Street Journal was first to report the news.

By CDR Joe Tenaglia, USN (Ret) – Four ships in four years – not to mention USS Antietam grounding and the USS Fitzgerald collision – apparently due to breakdowns in seamanship, something is wrong.
There is something wrong in the surface navy, this is the fourth seamanship disaster in as many years, with the loss of a USN minesweeper on a reef in the Phillipines, capture of two USN patrol boats in Iranian waters, now the collisions of two DDG’s, which are a mainstay of our sea based anti-missile capability at the height of tensions with North Korea (I could go on).
Slow things down, allowing time for junior surface officers to gain more experience and emphasis on basic seamanship skills may be an answer. They are expected to do too much too soon, and all the technology in the world does not solve that.
