Yes it was:
Theodore Arfsten, a quartermaster, remembered watching a Jewish officer cry when
he saw the blue Star of David on the planes' fuselages. At first, crew members below decks had no idea whose planes were shooting at their ship.

Simple friendly fire accident. Thousands friendly fire accidents happening in every war.
No, the aircraft had their insignia removed. That is an eye witness report I have seen.
The crew categorically stated that their communication frequency was jammed; as Nato and the Warsaw pact use different frequencies, the Israelis
had to know who they were attacking. All the crew always maintained that they thought it was no accident.
There was finally a US government report which said it was no accident.
Incidentally, a bizarre act was that fighter aircraft sent to defend them from the US fleet were recalled at the request of the US government. The admiral in charge was so shocked at this order that he actually asked to speak to the president himself to confirm it.
This was no accident.
'Fifteen years after the attack, an Israeli pilot approached Liberty survivors and then held extensive interviews with former Congressman Paul N. (Pete) McCloskey about his role. According to this senior Israeli lead pilot, he recognized the Liberty as American immediately, so informed his headquarters, and was told to ignore the American flag and continue his attack. He refused to do so and returned to base, where he was arrested.
Later, a dual-citizen Israeli major told survivors that he was in an Israeli war room where he heard that pilot's radio report. The attacking pilots and everyone in the Israeli war room knew that they were attacking an American ship, the major said. He recanted the statement only after he received threatening phone calls from Israel.'
And
'A former Navy attorney who helped lead the military investigation of the 1967 Israeli attack on the USS Liberty that killed 34 American servicemen says former President Lyndon Johnson and his defense secretary, Robert McNamara, ordered that the inquiry conclude the incident was an accident.
In a signed affidavit released at a Capitol Hill news conference, retired Capt. Ward Boston said Johnson and McNamara told those heading the Navy's inquiry to "conclude that the attack was a case of 'mistaken identity' despite overwhelming evidence to the contrary."'
Incidentally, do you know what Mcnamara said when interviewed on tv? 'No comment'.
