Pakistan got its nuclear technology from USA. AQ Khan got his centrifuge technology from NATO. AQ Khan was too junior to get the possession of documents of such high end on his own.

China is said to have exchanged the centrifuge technology of NATO in return for giving assistance in an older variant of atom bomb based on HEU. The bomb was quite heavy initially and was rather old technology which Pakistan could have made on its own once it got the centrifuge. So, the real fault lies with USA as centrifuge technology is more critical than the design of HEU bomb.

The fact is that China helped Pakistan only with civilian nuclear plants for power and energy. Nuclear weapon program was by Pakistan scientists. During 80's US was actively in the region, how could they miss it or ignore it had Pakistan was getting tot or assistance to build nuclear weapons? In reality US doubt on Pakistani scientists capabilities and didn't paid much attention internally in Pakistan but its eyes were scanning any assistance wrt nuclear weapons which could come from outside since Pakistani leaders have shown keen interest in these weapons.
