It wasn't always hindustan.the name hindustan was initially used for region between kabul and Delhi in 13th century and later other areas were added under its title eventhough they were not part of indus.
Hindh is corruption of the word sindh by persians and sindhu was the original identity of the region that comprises most of present day pakistan.
