Pakistan doesn't have to do much; If India continues on it's path of corruption, communal violence and lynching of significant minorities with the tactic of diverting the attention of its populace from its economic failure to Pakistani ISI pigeons, there's going to be a balkanization of Indian provinces anyways.

corruption has been normalized - it is juts another line item next to sales tax, so not a destabiling thing anymore.
communal violence - has mostly become desensitized as a media prompted thing, so while bad is not a destabilizing issue for India.
As you have pointed out astutely, only thing India has to worry about is the ISI pigeons. Rats are bad enough but rats with wings!

Lets Assume India captures GB and AJk which are obviously wettest of Hindutva dreams , how will they hold it ? Apparently they cant hold Jammu and Srinagar with massive army deployment. Pakistan will open IB front and let it escalate to point where international forces including India’s Daddy US will call to stop the conflict. World would nt want full fledged armed conflict between nations armed with nuclear weapons to teeth even though West’s interest align with India , what interests of west would be served in face of nuclear holocaust.
