Kind of when the first USAF Phantoms had both pilots in the cockpits.

I can't speak for the F111 crews but in the F-4, the US Navy never had rated pilots in the back seat and in the USAF, only early on, the back-seater was a pilot. In the Navy, they were called RIO's radar intercept officers, and in the air force, they were called WSO's weapon systems officers. Same job, different name and although both had some flight training in case of the pilot being killed or incapacitated, neither were rated pilots.
