Maybe they are not deliberately incompetent but they lack training and equipment to deal with terrorism. So Most of terror attacks are happening due to inside help. Why can't our intelligence agencies do the same and infiltrate these group. destroy them from inside.

political will (or lack of it) is the biggest issue (our politicians want to hug them and put flowers on them)
some very fine officers have lost their lives eventually despite their good performance and past wins against the terrorists

judiciary is another reason. our courts have been identified as the biggest hurdle in punishing the terrorist offenders
and finally its the shameless opinion makers who support the terrorists and blame the military operations as the cause for terrorism.

There are 1.5 million Bangladeshis and Pakistanis of Bengali origin ( not Bihari ) in Karachi and Sindh
