This report has clearly mentioned it's methodology, here let me paraphrase.
1) proportion of papers from that particular county in the top 10% most highly cited research reports
2) the H-index
3) the number of research institutions the country has in the world’s top 10–20 highest performing institutions.

this entire report is based upon research institution of a country not the products a country is producing.
the report is for measuring how much progress a country is making not how much research it already has under it's belt (this is the reason Russia is not in any of these reports).
A simple analogy would be Total GDP vs GDP growth rate.
Libya has a GDP growth rate of 17% but that doesn't mean they have a big GDP or they are a world class Economy.

Iran is in the report because not how much knowledge they already have but because how much progress they are making.
