As far as price for treatment is concerned. Pakistan lags behind India in multiple areas. Targeted Therapy, Immunotherapy have reached India many years ago. So a patient in India will receive the same treatment as a patient in the US minus any clinical trials.
Yes. There is also a huge disparity in treatment of people in rural and urban areas. India's approach has been very different. India for a long time now has started screening for cancer with mobile vans, camps and so on. I know so many people involved in this especially Karnataka and for Oral cancer.

We can compare density of cancer hospitals, screening, latest protocol of drugs used, hospice facilities, free treatment centers, doctors involved in international research, drug companies involved in India, etc.
