Lies. In Punjab we even call bhaiyyas madrassi. Everything South of Rajastan and East of Delhi is madrassi to us. It is very ignorant and should be corrected. In fact we don't even consider BIMARU states North Indian.

madras is the most famous city in southern india.. probably that's why people think anyone who speak southern language live in madras

The actual reason is :
Madras State was a former state in the Republic of India. At the time of its formation in 1950, it included the whole of present-day Tamil Nadu, Coastal Andhra and Rayalaseema, the Malabar region of North Kerala, and Bellary, South Canara and Udupi districts of Karnataka. Coastal Andhra and Rayalaseema were separated to form Andhra State in 1953, while South Canara and Bellary districts were merged with Mysore State, and Malabar District with the State of Travancore-Cochin in 1956. In 1969, Madras State was renamed Tamil Nadu, meaning "Tamil country".

Many old people in North India don't know Chennai, they still refer it as Madras.

That was way back in 60s and 70s when most of the mumbai underworld was controlled by South Indians like Haji Mastan, Varadarajan etc but not anymore. With the rise of Hindutva politics in 80s shiv sena dropped its anti south indian agenda along with most of its agendas against any hindu community.

Is Madrasi a racial slur, like the ch*nki used for NE Indians?

No, it is more like calling a South Indian a "Southie". I am not a South Indian but spent my childhood in Chennai and developed an accent and behavior like those from Chennai. After I left South India, many times people noted and told me that I talk and act like Madrassis - and each time it was all said and acknowledged very respectfully. Even when talking to people from Tamil Nadu (real Madrassis from the Madras of erstwhile), I too tend to some time say things like, "Madrassis are nice people"... or "Madrassis are dark"... or "You all Madrassis are too good at Chess..." and stuff like that. Never experienced the term being used as some sort of racial slur.

From what I have seen: -

Even calling Chinese/Mongoloid a Chinky (I suppose this is the word you were going to write) is not a racial slur, even when it is directed at a particular race and the intent too is outright abuse and provocation, because racism requires a mindset based on the false idea of "superiority" or "inferiority" of a particular race. When people call the Chinese "Chinky" or something on that line, they are indeed showing their hatred/phobia/ignorance about those people of that particular race, but the foundation of that hatred/dislike/phobia is not some sense of racial superiority. It is simply that - ignorance > phobia > dislike.

That does sound a bit like a racial slur.

Not a slur, almost complete South India was called as Madras till British Left India, that's why ppl from those areas are still referred as Madrassi, but it is dying down each day.

Nope... Not that much,But they still have some kind of obsessions with linguistic minorities.. eg-Attacks on NI workers(2008),Kannadigas in solapur(2009-2012), etc etc,They even targeted the largely Gujarati immigrants of maharastra in the very last state election...Aren't they Hindus??
They might have some reasons...Others are stealing their jobs and didn't respect Marathi language and culture..Actually its right to a certain extent..My only problem is with their so called NATIONALISTIC OUTLOOK tag...
