Unfortunately two massive factors are in play here.
The first is the ineffective if not blatantly incompetent leadership of our most important institution. Leadership at the top absolutely makes a difference. A leader unsure of himself and looking out for his personal goals like extensions will always place people around to gear the outcome towards that personal goal instead of putting the larger interests of the country at the forefront. This has a cascading impact down the institutional ladder. This is not unique to Pakistan Army but all institutions. There is a reason why the CEOs of all major Corporates are paid in gold.
The second factor is our absolute helplesness regarding Iran and this is where the Prime Minister comes in. Imran Khan has his "ideas" and he sticks to them. He has these fanciful notions about Afghans and Persians. The added factor recently is his recent spiritual journey and seeing himself as this Anti-War pacifist. The Persians are an old people in statecraft and no doubt have picked up on this weakness. According to Imran Khan

He has made up his mind and no amount of arguments or understanding that things are gray instead of black and white will change his mind.
This means that Iran the main backer of these terrorists will not be called out.
