In that case, it seems to be the pilot's mistake and we need to focus on their training and reduce mental stress...for example..working longer hours or under stress..It was not material failure or any specific part failure, in which case all remaining a/s are grounded.
It was a maneuvering accident in which the helo tail rotor came in contact with the ships super structure and crashed.
